1. About these Terms

These Terms of Use govern access to and use of www.synergyhealthfoundation.org and any pages, forms, content or services made available through it (the “Website”). The Website is operated by Synergy Health Foundation, Public Trust Registration No. E-12699/Thane, with its registered address at A4, 501/502, Vikas Complex, Castle Mill Compound, LBS Road, Opp. Holy Cross Convent High School, Thane (West) 400601, Maharashtra, India (“Foundation”, “we”, “us” or “our”).

By using the Website, you agree to these Terms. If you do not agree, please do not use the Website. Additional terms or consent notices may apply to a donation, medical camp, patient service, educational programme, volunteer activity or other specific service.

2. Charitable purpose and website information

The Foundation works to improve access to quality healthcare through medical education for healthcare professionals, patient education, awareness campaigns, medical camps and charitable medical or surgical initiatives. Website content is provided for general informational, educational and charitable purposes.

We aim to keep information accurate and current, but programmes, eligibility, locations, dates, clinicians, availability and other details may change. A Website announcement is not a promise that a programme, procedure, benefit or place will be available to any particular person.

3. No medical advice or emergency service

• Website content does not create a doctor-patient relationship and is not medical advice, diagnosis or treatment.
• Do not delay or disregard advice from a qualified healthcare professional because of Website content.
• The Website is not monitored as an emergency service. For a medical emergency, contact the appropriate local emergency service or go to the nearest suitable healthcare facility.
• Participation in a camp, procedure or patient programme may require separate screening, eligibility review, clinical assessment and informed consent.

4. Eligibility and submissions

You must provide information that is accurate, current and not misleading when using a contact, volunteer, medical camp or patient form. If you submit information for another person, you confirm that you are authorised to do so. A parent or lawful guardian should submit information and provide any required consent for a person under 18 years of age.

Submitting a form does not guarantee acceptance, selection, treatment, volunteering placement or a response within a particular time. We may request additional information where reasonably required for the relevant activity or legal compliance.

5. Acceptable use

You may use the Website only for lawful, personal, informational and non-commercial purposes consistent with these Terms. You must not:

• interfere with, damage, disable, overload or attempt unauthorised access to the Website, its servers, forms or data;
• upload malicious code, use automated extraction that materially burdens the Website, or circumvent security controls;
• impersonate another person, misrepresent authority, submit false medical or eligibility information, or use another person’s personal data without authority;
• use Website content or Foundation names, logos or materials in a misleading manner or to imply endorsement; or

6. Donations

Any donation made through or in connection with the Website is also governed by the Foundation’s Donation Policy. A donation is voluntary and does not purchase medical priority, treatment, influence, goods or services unless a separate lawful arrangement expressly says otherwise.

Unless otherwise stated, the Website and its original text, design, graphics, logos, educational materials and other content are owned by or licensed to the Foundation and are protected by applicable intellectual-property law. You may view and make a reasonable personal copy for non-commercial use, provided you do not remove notices or alter the meaning. Any other reproduction, adaptation, publication, distribution, commercial use or misleading use requires prior written permission or a valid legal exception.

8. Third-party content and links

Our Privacy Policy explains how we handle personal data submitted through contact, volunteer, medical camp and patient forms, as well as analytics and cookies. Specific health-data forms may also display a separate notice and consent request at the point of collection.
